However, there are concerns that the current diversity of medical services will be reduced and that the system will increasingly focus on fewer but larger centers, which could result in long travel distances and barriers to treatment for many patients. Increasing bureaucracy and competition for resources may also have negative effects, diverting focus away from patient care. One of the fundamental challenges in the patient-physician relationship is offering patients serious, comprehensible, and transparent care that is easy to understand. The hospital reform will indeed result in greater centralization of services, which can increase expertise and case numbers in specialized centers but also lead to accessibility issues. Especially in rural areas, long travel distances, limited appointment availability, and barriers in the healthcare system are already evident; centralization could exacerbate these problems. While the concentration of specialists undoubtedly brings advantages for rare and complex diseases, there is a risk that local care, particularly for elderly and chronically ill patients, will suffer.

Based on experiences and observations from other healthcare systems, such as in England, one can infer that developments in Germany may also move toward further centralization, which overall makes access more difficult. Despite these shortcomings, I view centralization as fundamentally positive because it improves experience and quality in treatment. However, the major challenge remains ensuring equal care for all patient groups – particularly for those who are not mobile or live alone. The discussion about central healthcare structures is complex: on the one hand, severely ill patients and those with rare diseases benefit from specialized centers; on the other hand, there is the risk that access for other patient groups will become increasingly restricted”, explains Prof. Dr. Sudhoff at the beginning of our conversation, adding:
