Increasing bureaucratization and competition for resources could also have negative effects, as they might divert attention away from patient care. The intended benefits of the reform are to improve the quality of care by promoting greater specialization and concentration in highly competent centers, thereby increasing treatment safety, strengthening medical expertise, and optimizing treatment for complex, rare diseases. The goal is to use resources more efficiently in order to achieve cost savings. For physicians, high visibility among patients is inherently important so that patients, with their personal medical histories, end up in the hands of those who can help them.

“Fundamentally, a challenge in the mutual relationship between patients and doctors lies in offering patients serious, comprehensible, and transparent care that is easy to understand. While the hospital reform will lead to greater centralization of services—which, on the one hand, can increase expertise and case volumes in specialized centers, but on the other hand, leads to accessibility issues—long travel distances, limited appointment availability, and barriers within the healthcare system are already being felt, particularly in rural areas; centralization could further exacerbate these problems. While the concentration of specialists undoubtedly offers advantages for rare and complex diseases, there is a risk that local care—especially for older and chronically ill patients—will suffer as a result.

Based on experience and observations of various healthcare systems, such as in England, it can be inferred that the trend in Germany could move toward further centralization, which would make access more difficult overall. Despite these shortcomings, I view centralization positively in principle because it improves the expertise and quality of treatment. However, the major challenge remains ensuring equal care for all patient groups—especially for people who are not mobile or who live alone. “The discussion surrounding centralized care structures is complex: On the one hand, critically ill patients and those with rare conditions benefit from specialized centers; on the other hand, there is a risk that care will become increasingly inaccessible for other patient groups,” explains Prof. Dr. Sudhoff at the start of our conversation, adding:
