Joint pain is a daily reality for many people. Whether it’s the result of a sports injury, age-related wear and tear, or chronic inflammation—when the knee aches or the shoulder hurts, the diagnosis is often complex. In the past, joints often had to be surgically opened up extensively to obtain an accurate diagnosis or provide treatment. Today, in many cases, invasive surgery is a thing of the past. The Principle of “Keyhole Surgery”

The term “arthroscopy” is derived from Greek (arthros = joint, skopein = to look). And that is precisely the principle: the surgeon looks inside the joint without having to open it completely. A high-resolution mini-camera and fine surgical instruments are inserted through tiny skin incisions, usually only 5 to 7 millimeters long.

The image from inside the body is highly magnified and transmitted to a monitor in the operating room. This allows the surgeon a level of precision that would be virtually impossible with the naked eye. Joint structures, cartilage surfaces, and ligaments can thus be assessed with great accuracy and often treated immediately during the same procedure.

From the knee to the little finger: Where is arthroscopy performed?

Originally, the technique was developed primarily for the knee joint, which remains its most common application today. Classic procedures include smoothing out meniscus tears, replacing the cruciate ligament, or treating cartilage defects. However, the technique has evolved rapidly. Thanks to increasingly precise instruments, arthroscopy is now possible on almost all joints:

  • Shoulder: For impingement syndromes, calcific tendinitis, or rotator cuff tears.

  • Ankle: To remove loose bodies or treat cartilage damage.

  • Elbow: For restricted movement or osteoarthritis (cubital osteoarthritis).

  • Hand and finger joints: Specialists can now perform minimally invasive surgery even on such delicate structures.

Diagnosis and Treatment in One Step

A key advantage of arthroscopy is that it combines diagnosis and treatment. When imaging techniques such as X-rays or MRI fail to identify a clear cause of symptoms, a direct view inside the joint provides clarity. If, for example, the surgeon finds a torn meniscus or an inflamed synovial flap, this damage can usually be repaired during the same anesthesia. This spares the patient a second procedure.

Back on Your Feet Faster

Compared to open surgery, arthroscopy is significantly less invasive (“minimally invasive”). Since no major muscles need to be cut, postoperative pain is usually less severe. The risk of infection is reduced, and the small scars are barely visible cosmetically. Particularly important for athletes and working professionals: rehabilitation is often faster. Many procedures can even be performed on an outpatient basis today, allowing patients to return home the same day. For minor procedures, so-called “early functional” postoperative care is often possible—meaning the joint can be moved again shortly after surgery, which prevents muscle atrophy and stiffness.

Conclusion: High-Tech for the Joints

Arthroscopy has permanently transformed orthopedics. It enables complex procedures with minimal trauma. Nevertheless, it is not a panacea: In cases of advanced osteoarthritis, when the joint is already severely damaged, even arthroscopy reaches its limits. In such cases, joint replacement (endoprosthetics) may be the better choice.
