The problem is that uric acid levels often rise over the course of several years before the first symptoms appear. During this time, crystals can already begin to deposit in the joints and tissues. Gout is also often associated with other conditions such as diabetes, high blood pressure, obesity, or lipid metabolism disorders.

In modern medicine, gout has therefore long since ceased to be viewed merely as a joint disease. Today, it is considered more of a metabolic disorder that can affect various organs.

How Gout Develops and the Role of Uric Acid

Uric acid is produced during the breakdown of purines. Purines are natural components of human cells as well as certain foods. Normally, excess uric acid is excreted by the kidneys, thereby maintaining a balance. However, if the concentration of uric acid in the blood rises persistently, small uric acid crystals form. These crystals tend to deposit in joints, tendons, or tissues. The immune system reacts to these crystals with severe inflammatory responses. These are the typical symptoms of an acute gout attack.

The most important risk factors include:

  • impaired kidney function
  • heavy alcohol consumption
  • a diet high in purines

The kidneys, in particular, play a central role. If uric acid excretion is insufficient, uric acid levels rise over the long term. Metabolic changes also significantly influence the risk. Modern research increasingly shows that insulin resistance and abdominal fat are closely linked to elevated uric acid levels. In addition, certain medications can affect uric acid excretion. These include, among others, some diuretics used to treat high blood pressure. 

Typical Symptoms

An acute gout attack often begins suddenly and develops within a few hours. Many patients report being awakened at night by severe pain.

  • Swelling
  • Tenderness
  • Limited range of motion

The metatarsophalangeal joint of the big toe is particularly commonly affected. However, the knees, ankles, fingers, or other joints can also become inflamed. The affected joints are often extremely tender. Even the slightest movements or light touch can sometimes cause severe pain. Without treatment, acute gout attacks usually last several days. As the disease progresses, episodes of inflammation occur more frequently and can affect multiple joints at the same time.

In addition, over time, uric acid crystals can also deposit outside the joints. This leads to the formation of what are known as tophi. These nodular deposits are often found on the fingers, toes, or earlobes. Recurrent joint inflammation, in particular, can cause long-term damage to the joints and significantly impair mobility and quality of life. 

How Gout Can Affect Metabolism and Organs

Modern medicine no longer views gout exclusively as a joint disease. Rather, it is often a complex metabolic disorder that affects numerous organ systems.

It is particularly often associated with:

  • High blood pressure
  • Diabetes mellitus
  • Obesity
  • Lipid metabolism disorders
  • Kidney disease

The kidneys themselves are also sensitive to elevated uric acid levels. Uric acid crystals promote the formation of kidney stones and can impair kidney function over the long term. In addition, recent studies have shown links between chronic inflammatory processes and cardiovascular diseases. Elevated uric acid levels may affect blood vessels and metabolism more significantly than previously thought. Inflammatory processes, in particular, play a key role in this context. Recurrent inflammation not only strains the joints but also affects the vascular system and metabolic regulation.

Being overweight further exacerbates the problem. Adipose tissue produces pro-inflammatory signaling molecules that can influence metabolism and immune responses. This is precisely why modern treatment approaches today do not focus solely on individual gout attacks but take into account the patient’s overall metabolic status.
