When Every Step Hurts: Modern Treatments for Ankle Osteoarthritis

The knee and hip are the “classic” sites of joint wear and tear. But the ankle joint also bears our entire body weight—and is more vulnerable than you might think. Ankle osteoarthritis (OSG osteoarthritis) often develops gradually, but in the long term leads to severe limitations in daily life. Whereas joint fusion was often considered the only option in the past, modern joint replacements now offer new possibilities for greater mobility.

Modern Treatments for Ankle Osteoarthritis

Unlike the Knee and Hip: The Causes

While osteoarthritis of the knee or hip is often age-related wear and tear, ankle osteoarthritis usually has a specific underlying cause. In about 80% of cases, it is a long-term consequence of an accident (post-traumatic osteoarthritis). This can be a complicated ankle fracture that occurred years ago, or chronic instability caused by frequent sprains (torn ligaments). The tricky part: Even if the injury seems to have healed long ago, the slightest misalignment or cartilage damage can lead to osteoarthritis over the course of years. The cartilage wears down, the joint space narrows, and the bones rub painfully against each other.

Typical Warning Signs

  • Start-up pain: The first few steps in the morning or after sitting for a long time feel stiff and painful.

  • Pain on exertion: After long walks, the ankle swells and hurts.

  • Limited mobility: Rolling the foot becomes increasingly difficult, often leading to a limping gait.

Treatment: Preserving the joint for as long as possible

In the early stages, orthopedic surgeons always try to preserve the joint. Conservative measures such as special orthotics, physical therapy, or anti-inflammatory injections (e.g., hyaluronic acid or cortisone) can often provide long-term relief from symptoms. Arthroscopy (joint endoscopy) can also help by removing irritating bone spurs or inflamed tissue through a minimally invasive procedure (“joint debridement”).

When all else fails: Fusion or a prosthesis?

If the cartilage is completely destroyed (Grade 4 osteoarthritis), patients face a decision: fusion (arthrodesis) or an artificial joint (prosthesis)?

  1. Fusion (arthrodesis): This is still considered the “gold standard” for long-term pain relief and high load-bearing capacity. In this procedure, the tibia and talus are firmly fused together. Advantage: The joint is extremely stable and pain-free. Patients are usually able to walk normally again, as the adjacent ankle joints partially compensate for the loss of movement. Disadvantage: Mobility in the upper ankle joint is lost. In the long term, this can lead to stress-related damage in the adjacent joints.

  2. The ankle prosthesis: Modern three-component prostheses have revolutionized treatment. They consist of metal components for the tibia and talus, as well as a smooth-gliding plastic core in between. Advantage: The natural gait pattern is preserved. The adjacent joints are spared. Suitable for: Patients with good bone quality who do not perform heavy physical labor but wish to maintain their mobility (e.g., for hiking or cycling).

Conclusion: An Individual Decision

There is no single “best” solution. Whether fusion or a prosthesis is more suitable depends on age, activity level, and bone quality. Today, specialized foot surgeons can often offer customized solutions so that patients can once again feel solid ground beneath their feet—without pain with every step.
